All About Iron

Iron Biochemistry

Many know it's a component of hemoglobin, and is therefore essential for the delivery of oxygen.


It's part of cytochromes in the electron-transport chain and plays a role in mitochondrial energy production.

Iron is required for the synthesis of thyroid hormone.


It's also needed for the conversion of tyrosine to dopamine.


Lastly, iron plays a role in immune function.


Because of its above roles, we always screen our Edmonton patients with fatigue for iron deficiency.


Iron Absorption

Heme iron (found in meat, poultry, and fish) is well absorbed, a 15–35% absorption rate.


Nonheme iron (from plant foods and in supplements) has 2–20% absorption. 


Also, nonheme iron is inhibited by substances in plant foods like phytates and polyphenols.


Vitamin C enhances the absorption of nonheme iron.


Stomach acid is needed to transform ferric (Fe2+) iron to ferrous (Fe3+) iron.


We work with a number of highly absorbable forms of iron therapy at our Edmonton office.

Measuring Iron Levels

No one test provides perfectly reliable information about iron status.


In most cases, a combination of tests gives a more accurate picture than does any single test.


Ferritin levels are the main measurement we use for iron status.


Serum iron (as well as saturation index and TIBC) are also helpful.


Complete blood count (with attention to hemoglobin and MCV) are also beneficial.


The practitioners at TruMed work with all of these tests but rely mostly on ferritin levels.


Iron Deficiency Therapy

Most patients are treated with OTC irοո because it is generally effective, and inexpensive.


However, up to 70 percent of patients report gastrointestinal side effects from standard iron.

In our Edmonton patients we frequently see regular iron ineffective in rectifying low levels.

Iron Infusions in Edmonton

TruMed has been proudly offering IV therapy in Edmonton since 2010.

In 2025, we added Iron Infusions to our suite of IV therapy options for our Edmonton patients.

Some patients refer to this as an iron transfusion - but it's actually an iron infusion.

IV Iron can dramatically increase ferritin levels - it is common to see ferritin rise by 100 to 200 points after single infusion.

When giving Iron IVs - our staff takes extreme precautions to ensure maximum safety and tolerance.

We are one of the only private providers of Iron Infusions in Edmonton.

An average Iron Infusion takes about 60 minutes to administer.

There are two types of iron infusions that are available in Canada and therefore Edmonton: Monoferric and Venofer.

Monoferric (Ferric derisomaltose) and Venofer (Iron sucrose) -  these Irons used for infusion differ from iron for oral consumption which is generally bound to another substance "chelates". Iron suitable for intravenous injection is a iron hydroxide core with an envelope formed by iron–carbohydrate particles. These two types of IV Iron differ depending on their molecular weight.

Iron injections Edmonton.

At our Edmonton office, we offer both types of Irons for infusion.

Generally speaking, Monoferric is preferred because the dose per infusion is much higher than Venofer. So most of what we inject at our office is Monoferric Iron.

Monoferric is the most commonly administered injectable iron. Venofer in Canada is almost exclusively used in pregnancy. 

Products differ from each other by the size of their core and the identity and density of
the surrounding carbohydrates. Carbohydrates stabilize the gel, slow the release of iron
and maintain the resulting particles in colloid suspension.

Although the impact on Iron levels can be quite dramatic there are some potential side effects to Iron IV Therapy.

In about 5% of patients, a flushing reaction is experienced with chest tightness, shortness of breath and back tightness. Although this tends to dissipate is slowed down.

There is a small risk of anaphylaxis with Iron IV Therapy: between .002 and .004% depending on the type of Intravenous Iron given.

IV Iron can also have a range of delayed side effects such as: hives, rashes, joint swelling, joint pain, fever, flu-like symptoms, abdominal pain, diarrhea and constipation as well as muscle pain and cramping. Although we have not seen much of this in most of our patients at our Edmonton office we still like to let patients know that it is possible to experience these things when getting an Iron IV.

Generally speaking, we dose IV Iron (monoferric) at 500-1000mg per dose based off of your original ferritin level as well as hemoglobin level. For iron infusions, we use the Ganzoni formula to choose the dose range for our Edmonton patients.

We do not do Iron Infusions in patients with Thalassemia unless there is a documented low ferritin as well as males under the age of 18.

It is important not to dislodge the IV, as IV Iron can stain the skin permanently.

After an Iron Infusion, you will be sent for blood work in about 4 to 6 weeks. 

Most patients will only require one to two IV Iron treatments using a preparation of Intravenous Iron called Monoferric.

Pregnant women can also do Intravenous Iron - the dose is typically spread out over a handful of treatments using the type of iron called Venofer.

You do not require a prescription for Intravenous Iron to be able to get an infusion at our office in Edmonton. Our Nurse Practitioner on staff will evaluate your case and can prescribe the Iron Infusion for you.
